CalFresh Healthy Living, California’s SNAP-Ed program, developed the Community Impact Framework in 2023 to strengthen the reach of nutrition and physical activity efforts across the state. In 2025, federal funding for SNAP-Ed was eliminated, leading to the sunsetting of the CalFresh Healthy Living program.

The Community Impact Framework was created through a collaborative process led by a Local Advisory Council, with backbone support from Leah’s Pantry. Local and state staff, including subcontractors, provided feedback throughout the process.

CalFresh Healthy Living is California’s Supplemental Nutrition Education (SNAP-Ed) program. It supports people of all ages who qualify for CalFresh, live in households with incomes at or below 200 percent of the federal poverty level, or participate in other federal assistance programs. This document serves as an addendum to the Federal Fiscal Year (FFY) 2024 – 2026 California Department of Public Health (CDPH) CalFresh Healthy Living Funding Application Package and supersedes the requirements and deliverables therein. The purpose of Program Letter (PL) 25-06 is to provide FFY 2025 and 2026 requirements and guidance for Local Health Departments (LHDs) to complete and close out local CalFresh Healthy Living programs.

This five-stage plan offers ideas to guide the closeout of your direct education program and can be adapted to fit your timeline and objectives. The concepts, tasks, and materials included are meant to inspire your transition planning. They should not replace guidance from your State Implementing Agency or program management.

The Partnership Agreement is a sample template that outlines roles and expectations as programs transition from direct education to teacher-led or site-led delivery. It provides a clear, customizable framework to support shared understanding between CFHL staff and partners. Sites can adapt the sample agreement to fit their specific needs, capacity, and program structure.

This supplemental handout is all about flexibility and creativity. It encourages teachers and partners to make nutrition and physical activity lessons their own.

The Sustainability Self-Assessment Tool is a sample worksheet that helps sites reflect on their readiness to continue nutrition education independently. It highlights key areas such as teacher confidence, resource access, integration into school routines, and champion support. Programs can adapt this sample tool to match their unique goals, structures, and sustainability needs.
